Our Mission

Independent pharmacies serve more diabetic patients than almost any other healthcare setting — but they have no way to capture the chronic care management revenue that hospitals and large health systems collect. GlycoSphere changes that.

We operate clinical chronic care programs inside independent pharmacies. We bring trained staff, FDA-cleared devices, a contracted billing provider, and a purpose-built technology platform. The pharmacy provides the space. We handle everything else.

How We're Different

We Bring the Staff

Our salaried clinical coordinators are trained in RPM workflows and rotate across partner pharmacy locations.

We Bring the Provider

Our contracted NP/PA provides medical supervision and billing sign-off, so the pharmacy never needs to hire a physician.

We Bring the Technology

FDA-cleared cellular monitoring devices from Tenovi plus our HIPAA-compliant care management platform.

Our Story

GlycoSphere was built to solve a structural problem in healthcare: independent pharmacies are the most accessible healthcare touchpoint for millions of chronic disease patients, yet they have no pathway to participate in Medicare's fastest-growing reimbursement programs — Remote Patient Monitoring and Chronic Care Management.

The barrier isn't clinical capability. Pharmacists are already trusted medication experts who see their patients more frequently than any other provider. The barrier is infrastructure — the billing provider relationship, the monitoring technology, the compliance documentation, and the trained staff to deliver it all.

GlycoSphere removes every barrier. We place our own clinical team inside partner pharmacies, equip them with FDA-cleared cellular devices, connect them to a supervising billing provider, and automate the entire documentation and billing workflow. The pharmacy provides the space. We handle everything else.
